1. Fertility services and medication benefits are subject to any applicable deductible, copay, or coinsurance as described by your Aetna medical plan. Note: Any prior fertility and medication benefits used under your prior plan through United Healthcare will count towards the $60,000 lifetime maximum.
2. Fertility services and medications are covered under your respective Kaiser and HMSA plans, please verify specifics with your medical carrier.
3. The donor, surrogacy, and adoption benefit is designed to reimburse for expenses unrelated to actual treatment that would be covered through your major medical or fertility plan. Eligible expenses reimbursed by Kindbody on Tesla’s behalf are considered taxable wages to you.
4. Effective 1/1/25, the lifetime maximum for fertility services and required medications will be $60,000 combined. For the remainder of 2024, it will remain broken out as $40,000 for fertility services and $20,000 for fertility medications.
